Reporting Major Unusual Incidents (MUI)

A Major Unusual Incident (MUI) is defined as any alleged, suspected, or actual occurrence of an incident that adversely affects the health and welfare of an individual receiving services from the County Board. There are nineteen types of incidents defined in the rule that are considered major unusual incidents. All of these require an investigation in compliance with the Ohio Department of Developmental Disability rule standards.
The Meigs County Board of Developmental Disabilities contracts with the Southern Ohio Council of Governments to investigate all MUIs and ensure that immediate actions are taken to protect individuals from harm. An investigation is conducted to determine the
cause and contributing factors of the incident and develop a prevention plan to reduce the likelihood of recurrence.
Actual or suspected MUIs may be reported to the Meigs County Board of Developmental Disabilities by calling (740) 992-6681 during business hours or by calling (740) 508-8790 during non-business hours.
